Client Contract 

Abbey Daw, For Her Dream LLC, Effective Date: May 1, 2023
Neuro Somatic Breath Therapy & Intuitive Reiki Healing 

In consideration of being accepted to be Abbey Daw’s client and partake in the practices, coaching, and all therapeutic tools and or exercises owned by and facilitated by For Her Dream, LLC and Abbey Daw. The undersigned, on behalf of him/herself/themself, his, her or their heirs, executors, administrators, assigns, personal representatives, minors, knowingly and voluntarily enters this Waiver, Release of Liability and Agreement to Indemnify and Hold Harless and to Defend (hereafter the “waiver). 

I hereby FOREVER RELEASE AND DISCHARGE For Her Dream, LLC and owner Abbey Daw, its partners, staff, administrators, employees, facilitators, agents, affiliates, volunteers, representatives, and assigns (the “Released Parties”) from all liabilities, claims, demands or causes of action that I may hereafter have for personal injuries, death or property damage arising out of my use of the facilities or as a client of Abbey Daw. I hereby acknowledge that For Her Dream, LLC and Abbey Daw cannot guarantee that the signed client’s life will change and can only promise if the client chooses to participate in these healing modalities, shifts can occur to help support one’s life to healing, happiness, and transformation. 

I am fully aware of all the risks associated with therapeutic work and I am qualified, in good health, and in proper physical condition to participate in healing and activities prescribed by Abbey Daw owner of For Her Dream, LLC.  Should anything happen during this time working with Abbey Daw owner of For Her Dream, LLC. Is not held liable. 

I further agree that Abbey Daw is not a licensed psychologist, LCSW, or psychiatrist and would be recommended for any mental health-related disorder, the client consults with a doctor before participating. Abbey Daw cannot prescribe any medicine related to the client’s mental health condition. Abbey Daw can only provide the necessary therapeutic tools to help the client improve their mental health and well-being. 

I agree that this Waiver shall be construed in accordance with and governed by the laws of the State of Utah.  In addition, I agree that all litigation, mediation, or arbitration for property damage, personal injury, or related loss against any of the released parties must be brought and obtained in Salt Lake County, Utah. Further, I consent and agree that the jurisdiction and venue of any litigation shall lie exclusively in state and federal courts located in Salt Lake County, Utah. In the event any portion of this Waiver is found to be unenforceable, the remaining terms shall be fully enforceable to the greatest extent permitted by the law.

By purchasing these products and services I agree that I am 18 years old or older, or have the approval of a legal guardian 18 years or older, and release any and all liability from Abbey Daw, For Her Dream LLC, and all of its associates.
